Neutralize background if post processing in Pixinsight?

Just wondering if this should be enabled if I'm doing all my post processing in Pixinsight?  Are there downsides to disabling it in APP when integrating?

Yes, I wouldn't do that as it's part of the calibration of your data. APP is very good at these things, in my opinion better than PI (at least more consistent without you having to do a lot of tweaking). Post-processing I would do on the final integrated result. Just fully integrate it in APP, then if preferred post-process that still unstretched stack in PI.
